Certificate IV in Home and Community Care (CHC40208)

Overview

This course has been designed to give participants the knowledge, understanding and practical skills needed to provide services to individuals with complex needs and/or work with older people in a nursing home, hostel or home care environment.

On successful completion participants will receive a Nationally Recognised Certificate IV in Home & Community Care (CHC40208).

Prerequisites

Competency in 3 units from Certificate III in Home & Community Care (CHC30308) or equivalent.

• CHCAC318A: Work effectively with older people
• CHCDIS301A: Work effectively with people with a disability
• CHCICS304A: Work effectively with carers
   

Course Content

Includes 15 units of competency (in addition to the 3 prerequisite units) covering:

• Provide services to older persons with complex needs
• Implement interventions with older people at risk of falls
• Coordinate and monitor home based support
• Supervise work
• Coordinate information systems
• Facilitate individualised plans
• Facilitate support responsive to specific nature of dementia

Learning Outcomes

On completion of the course, participants will be able to:

• Carry out activities related to maintaining an individual’s well being through personal care and /or other activities of living
• Provide services to individuals with complex needs, and / or work with groups of older people
